Ruralidad y mortalidad prematura por cáncer de páncreas: revisión narrativa

Resumen

Introducción: La ruralidad puede condicionar el acceso oportuno al diagnóstico y tratamiento del cáncer de páncreas, pero la evidencia específica sobre mortalidad prematura es escasa y no debe confundirse con mortalidad global o supervivencia. Objetivo: Analizar críticamente la asociación entre ruralidad y mortalidad prematura por cáncer de páncreas, distinguiendo la evidencia directa de los desenlaces indirectos relacionados con supervivencia, estadio, cirugía y acceso terapéutico. Métodos: Se realizó una revisión narrativa estructurada de publicaciones entre enero de 2000 y junio de 2026 en PubMed/MEDLINE, Scopus, Web of Science y Google Scholar. Se identificaron 36 registros, 18 fueron evaluados a texto completo y 12 estudios se incluyeron en la síntesis comparativa. La mortalidad prematura se interpretó como muerte entre los 30 y 69 años, de acuerdo con el marco de mortalidad prematura por enfermedades no transmisibles de la Organización Mundial de la Salud. Resultados: Ninguno de los 12 estudios priorizados evaluó como desenlace principal la muerte antes de los 70 años. La evidencia disponible fue, por tanto, predominantemente indirecta. Los residentes rurales mostraron menor probabilidad de diagnóstico temprano y de acceso a tratamiento potencialmente curativo. Se notificaron menor resección (OR=0,82; IC95%: 0,69–0,99), menor tratamiento multimodal (OR=0,70; IC95%: 0,38–0,97) y diferencias de mortalidad o supervivencia que, en varios estudios, se atenuaron después del ajuste por nivel socioeconómico, estadio, tratamiento y tipo de centro. Conclusiones: La ruralidad se asocia con desigualdades relevantes en la trayectoria asistencial del cáncer de páncreas; sin embargo, la evidencia disponible no permite afirmar un efecto causal independiente sobre la mortalidad prematura. Se requieren estudios poblacionales que midan de manera explícita las defunciones entre 30 y 69 años y que integren variables clínicas, socioeconómicas y territoriales.
